Understanding Your Partners in IRIS

 Your TMG IRIS Consultant Will:

  • Listen to what you want your life to be like and help you figure out how to meet your goals.
  • Help you understand how to self-direct in IRIS.
  • Help develop your IRIS plan to fit in your budget.
  • Make sure all required paperwork is done.
  • Assist you to complete new employer and employee paperwork and send these to your fiscal employer agent (FEA).
  • Help you explore different jobs.
  • Help you find workers, providers, and places to buy equipment and supplies.
  • Review how well your plan is working.
  • Help you create a backup plan so that you are safe.
  • Help you learn about and understand your responsibility to stay eligible for Medicaid and IRIS.
  • Contact you monthly, meet you in-person at least quarterly, and visit you once a year in your home.

 Your TMG Long-Term Care Functional Screener Will:

  • Meet with you in your home once a year to complete the Long-Term Care (LTC) Functional Screen to make sure you’re still eligible for IRIS.
  • Listen and gather information from you, your health care providers and others who support you.
  • Accurately show your strengths, abilities and support needs when completing the LTC Functional Screen.
  • Meet with you more than once a year if you have a change in your health condition that impacts your ongoing daily support needs.

 Enrolled in IRIS Self-Directed Personal Care (SDPC) Program? Your IRIS SDPC Registered Nurse Will:

  • Review your care needs and determines your hours of care.
  • Provide oversight to help you in self-directing your personal care services. The SDPC Nurse does not provide nursing services, like reviewing medications or taking blood pressure.
  • Work with your doctor to get orders for your personal care and to ensure services are authorized before you receive them.
  • Partner with you to develop your person-centered plan of care called “My Cares”, which you use to train and supervise your personal care workers.
  • Provide training and education to workers you hire or to you, so that you can train your workers.
  • Visit your home every 60 days to answer questions, make sure your My Cares plan is being followed, and ensure there are no health and safety concerns.

 Your Fiscal Employer Agent (FEA) Will:

  • Complete all of the employer and employee setup tasks, including the criminal and caregiver background checks on workers you may hire.
  • Collect your monthly Medicaid cost share, if you’re required to pay one.
  • Help to verify provider qualifications and help you make written provider agreements.
  • Send payments you authorize to the business(es) or agency listed on your plan that provide you with goods, supports and services.
  • Process paperwork and time sheets when you hire your own workers.
  • Contact you and your IRIS Consultant Agency (ICA) if you ask for something to be paid that’s not in your plan.
