TMG knows that people want to live at home and in their communities as long as they can. We believe that when people need help from others to get dressed, make meals, go to the bathroom, or get to the places they want to go, they should be able receive the help they need to stay in their homes as long as possible.

 

Home and Community-Based Waiver (HCBW) Services help people live at home instead of moving to institutional settings like nursing homes. People in waiver programs receive the supports and services they need to live at home and be a part of their communities. People must be financially eligible for Medicaid to qualify for these programs.

 

Quality Assurance and Quality Improvement (QA/QI) Services TMG has a contract with the Wisconsin Department of Health Services (DHS) to provide quality assurance and quality improvement (QA/QI) services. We work with the legacy waiver program that serves adults with physical disabilities and older adults. We also provide QA/QI services for the children’s waiver program.

 

Quality Services Specialists work with local waiver agencies to enroll people in the waiver program. We review applications and records to make sure they are completed correctly. We attend home visits to meet people who need assistance to help them remain in their home with the cares and supports they need. We check to see if they have health or safety concerns and how satisfied they are with the help they receive. We help care managers find answers to complicated situations.
